Un fallo en la computadora fue la causa del accidente de Conviasa hace 5 años en Puerto Ordaz

La tragedia que le costó la vida a 17 personas y causó heridas a 34 al estrellarse un avión ATR 42 de la aerolínea Conviasa el 13 de septiembre de 2010 en Puerto Ordaz, fue causado por un fallo en el sistema de alerta de la tripulación, según reveló el informe oficial de la investigación sobre este accidente.

La Dirección General para la Prevención e Investigación de Accidentes Aéreos del Ministerio del Poder Popular para el Transporte Acuático y Aéreo (DGPIAAE) emitió el documento final en el que determinan lo antes dicho como “causa más probable”, ya que la grabadora de datos de vuelo del avión siniestrado no estaba funcionando para el momento del hecho, por lo que no pudo obtenerse información sobre estos parámetros.

“La DGPIAAE determina que la causa más probable para la ocurrencia del accidente fue el mal funcionamiento del sistema centralizado de alerta de tripulación (CCAS/CAC) con activación errónea del sistema de advertencia de pérdida de sustentación de vuelo”, dice el informe.

El texto oficial añade que “como factor contribuyente, deficiencias en la gestión de los recursos de cabina de la tripulación, pérdida de la conciencia situacional, inadecuada coordinación durante el proceso de toma de decisiones para enfrentar situaciones anormales en vuelo, desconocimiento del sistema de advertencia de pérdida y manipulación inadecuada de los controles de vuelo”.

Factor técnico y humano

El informe de la DGPIAAE, ente adscrito al Ministerio de Transporte Acuático y Aéreo, detalló que el avión fue volado “con dos condiciones anormales, activación del sistema de advertencia de pérdida y el desacople de las superficies de control transversal o elevadores (PUM) de la aeronave, requiriendo un esfuerzo constante por parte del piloto al mando para mantener el control de la aeronave”.

Asimismo indica que existió una “manipulación inadecuada de los compensadores en la fase final del aterrizaje, que conllevó a que el piloto al mando ejerciera mayor esfuerzo sobre el control de vuelo antes del impacto”.

De igual manera señala que “el piloto al mando presentó fallas a nivel cognitivo-emocional y a nivel de sus habilidades, exteriorizó falta de liderazgo y errores de juicio que lo llevaron a tomar decisiones desacertadas”.

El documento oficial concluye también que “ambos pilotos mostraron confusión, pobre coordinación en la cabina de vuelo, fallas graves en comunicación, falta de conocimiento en los sistemas de la aeronave y pérdida de conciencia situacional”.
De lo anterior se deduce que la falla técnica sufrida por la aeronave condujo a una serie de errores en la selección y aplicación de los procedimientos a seguir por parte de la tripulación del ATR 42 siglas YV-1010 de Conviasa, factores que determinaron el trágico desenlace del vuelo 2350.

En base a ello, el informe recomienda establecer o reforzar los procedimientos de formación, evaluación y supervisión de las tripulaciones, con especial énfasis en el desarrollo de sus capacidades de comunicación, conciencia situacional e utilización de los recursos de cabina.

Además, insta a reforzar los procedimientos de instrucción y evaluación de habilidades relativas al sistema de control de vuelo y sistemas de advertencia de los aviones ATR 42.

La información emitida por este informe es parte del documental independiente “Puerto Ordaz 2350”, realizado por un equipo de periodistas de Ciudad Guayana que es difundido por entregas a través de la página web de videos Youtube durante esta semana al cumplirse cinco años del accidente.

A dicho documento oficial conclusivo se tuvo acceso gracias al asesor de víctimas de accidentes aéreos William Dugarte, quien representó a un grupo de familias afectadas por la tragedia, y es uno de los entrevistados en “Puerto Ordaz” 2350.

Description: The Aka Pygmies living in the south-west region of the Central African Republic have developed a distinctive vocal musical tradition, which involves a complex type of contrapuntal polyphony based on four voices, mastered by all members of the Aka community.
Music and dance form an integral part of Aka rituals including ceremonies related to the inauguration of new encampments, hunting and funerals. Unlike polyphonic systems that are written down in notation, the vocal tradition of the Aka Pygmies allows for spontaneous expression and improvisation. During performances, each singer can change his or her voice to produce a multitude of variations, creating the impression that the music is continuously evolving. The songs are generally accompanied by various percussion and string instruments, each one played for a specific occasion.Among the most common instruments are a local type of drum (enzeko), a harp-like instrument known as the geedale-bagongo, and the single-string bow (mbela). The songs perpetuate essential knowledge for the cohesion of the group and the preservation of community values. The dances are performed to the accompaniment of vibrant hand-clapping. Depending on the ritual, some dances feature men only, while others may be executed by couples or by male and female solo dancers. Relying entirely on oral transmission, the Aka Pygmies have succeeded in preserving their musical knowledge within the community by including children in rituals from an early age.
The lifestyle of the Aka Pygmies has been drastically disrupted due to the changes currently taking place in the Central African Republic. The scarcity of game resulting from deforestation, the rural exodus and the folklorization of their heritage for the tourist industry are the principal factors contributing to the gradual disappearance of many of their traditional customs, rituals and skills.
